Question 2: What was the date of his admission into the Hospital?
Answer: Here LIC wants to know that if the Policyholder was admitted to the hospital, then when was he last admitted as per the hospital records.

Answer: The answer to this question may be different for different patients. The hospital has to tell all the dates when the policyholder was admitted, the duration for which he was admitted and the symptoms of the policyholder's illness as per its records.
Question 5(b): Was the history reported by the patient himself or by someone else?
Answer: The hospital should tell whether the patient told all his problems himself or by someone else.
Question 5(c): If the history was not reported by the patient himself, the name and relationship of the person who reported. Was the patient present at that time and was he conscious?

Question 6: What was the diagnosis arrived at in the Hospital?
Answer: The hospital records the answer to this question as per its records.
Question 7: Was there any other disease or illness which preceded or co-existed with the ailment at the time of the patient's admission into the hospital? If so, what was it? Please give details stating:

- Date when first observed by the patient
- By whom treated?

Answer: In many cases, some patients are admitted to the hospital due to one major problem, but they may also be suffering from some other diseases. Question 8: What was the date of his discharge from Hospital?
Answer: The answers to this question may also vary in different cases. Some patients die during treatment itself. So after death, when the body is handed over to the family, that date should be written here. In some cases, the patient is discharged after being considered healthy, but dies a few days after reaching home. So in such cases, the date on which he was discharged should be noted here.
Question 9: What was his condition when he was discharged?
Answer: If the patient was in a normal state of health at the time of discharge but needed treatment, the hospital fills in the details according to the patient's condition. If the patient's body was handed over after his death, then the answer to this question should be "deceased".
