    • Did you see the body after death: If you saw the body of the deceased policyholder in open form, then answer yes, otherwise write no.
    • Was the body buried or cremated: Answer this question according to the actual situation.
    • Time and Date of burial or cremation: The date should be written clearly in the answer to this question, but it is better to write the time with the word "approximately".
    • Name and address of the place of burial/cremation: Wherever the deceased policyholder was buried or cremated, the name and full address of that place should be written clearly.
    • Were you present at the disposal of the body: Remember, the answer to this question should always be yes.
